I am sure there is a way to do it with the Squarespace form via some custom script or code but the easier way might be to build a form yourself and paste it via the Code block. You could also use something like Jotform (http://www.jotform.ca/) or Wofoo (http://www.wufoo.com/) which have built in payment features.
